About the conference
The conference will focus on current trends in long-term coordinated care, present examples of best practice from regions and local authorities, and introduce specific methodological tools used in client-centred work. Its content is designed to link strategic perspectives with frontline practice and to create space for sharing experience among professionals from different countries and sectors.
Attention will be given to interdisciplinary collaboration and to the role of the case manager in coordinating care across different types of services, including opportunities to individualise support in line with clients’ needs. The programme will also address service quality and accessibility, financing, and regulatory frameworks, all considered in the context of demographic and societal changes that are shaping the further development of coordinated care.
The conference builds on the long-standing experience of Sue Ryder in implementing the principles of coordinated care both in practice and at the systemic level. The content will include panel discussions, interactive workshops, and case studies from practice, offering participants concrete inspiration for applying case management principles within their own organisations and systems.
